Switch to the Mortgage worksheet, then format the headings. Merge and center the range A1:D1. Apply the FM Heading cell style to the merged cell.

Navigate to the "Page Layout" tab. "Themes" must be right below the "File" tab. Expand it and choose the "Office" theme.
Select range A1:D1, switch back to the "Home" tab. Click "Merge & Center" which is in the alignment group. After that, click "Cell Styles" from the styles group and choose FM Heading.
Merge & Center to the range A2:D2, click "Cell Styles" and select FM Subheading.
Select the range A4:B4, click "Cell Styles" and choose Note cell style which is in the Data and Model section.
Select range A6:D6, click the center option from the Alignment group and bold option from the font group. At the font group, expand the font color option. Choose Blue-Gray, Text 2 which is in 4th column, 1st row of the Theme Colors palette. After that, select A7 through A9 and select Merge & Center from Alignment group.
